Si la robotique de r\u00e9\u00e9ducation, qui facilite les mouvements pendant la th\u00e9rapie, a am\u00e9lior\u00e9 l&rsquo;entra\u00eenement des patients, elle est limit\u00e9e dans sa capacit\u00e9 \u00e0 r\u00e9activer le syst\u00e8me nerveux. Sans engagement musculaire actif, la th\u00e9rapie robotique seule ne permet pas une r\u00e9cup\u00e9ration \u00e0 long terme.<\/p>\n\n\n\n<p>Une \u00e9quipe dirig\u00e9e par les professeurs Gr\u00e9goire Courtine et Jocelyne Bloch a mis au point un syst\u00e8me enti\u00e8rement int\u00e9gr\u00e9 qui combine une neuroproth\u00e8se implant\u00e9e dans la moelle \u00e9pini\u00e8re et la r\u00e9\u00e9ducation robotique. Cette approche d\u00e9livre des impulsions \u00e9lectriques pr\u00e9cises pour stimuler les muscles en synchronisation avec les mouvements robotiques, favorisant ainsi une activit\u00e9 naturelle et coordonn\u00e9e. Le projet a \u00e9galement mis \u00e0 profit l&rsquo;expertise en robotique du <a href=\"https:\/\/www.epfl.ch\/labs\/biorob\/\">laboratoire du professeur Auke Ijspeert \u00e0 l&rsquo;EPFL<\/a>, assurant une synchronisation parfaite entre la stimulation spinale et l&rsquo;assistance robotique.<\/p>\n\n\n\n<p>Contrairement \u00e0 la stimulation \u00e9lectrique fonctionnelle conventionnelle, la stimulation \u00e9pidurale biomim\u00e9tique utilis\u00e9e dans ce syst\u00e8me imite les signaux nerveux naturels, activant ainsi les motoneurones plus efficacement. Des capteurs sans fil d\u00e9tectent les mouvements des membres en temps r\u00e9el, ajustant automatiquement la stimulation pour correspondre \u00e0 chaque phase de mouvement. Cela garantit une exp\u00e9rience de r\u00e9\u00e9ducation fluide et adaptative, b\u00e9n\u00e9fique \u00e0 la fois pour la th\u00e9rapie clinique et les activit\u00e9s quotidiennes.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Changer les normes de r\u00e9\u00e9ducation<\/h4>\n\n\n\n<p>L&rsquo;\u00e9quipe a test\u00e9 son approche avec cinq personnes souffrant de l\u00e9sions de la moelle \u00e9pini\u00e8re. Les r\u00e9sultats ont montr\u00e9 une activation musculaire imm\u00e9diate et durable, les participants retrouvant des mouvements volontaires m\u00eame apr\u00e8s l&rsquo;arr\u00eat de la stimulation. Au-del\u00e0 des param\u00e8tres contr\u00f4l\u00e9s, les participants ont utilis\u00e9 avec succ\u00e8s le syst\u00e8me pour marcher avec un d\u00e9ambulateur et faire du v\u00e9lo \u00e0 l&rsquo;ext\u00e9rieur, prouvant ainsi son efficacit\u00e9 dans des conditions r\u00e9elles.<\/p>\n\n\n\n<p>Afin de garantir une large adoption clinique, les chercheurs ont travaill\u00e9 avec plusieurs centres de r\u00e9\u00e9ducation pour int\u00e9grer leur technologie dans des syst\u00e8mes robotiques largement utilis\u00e9s, notamment des tapis de course, des exosquelettes et des v\u00e9los stationnaires.<\/p>\n\n\n\n<p>\u00ab Nous avons visit\u00e9 des centres de r\u00e9\u00e9ducation pour tester notre technologie de stimulation avec leurs syst\u00e8mes robotiques, et il \u00e9tait incroyablement gratifiant de voir leur enthousiasme \u00bb, expliquent Nicolas Hankov, chercheur chez <a href=\"https:\/\/www.neurorestore.swiss\/\">.NeuroRestore<\/a>, et Miroslav Caban, chercheur chez BioRob. \u00ab Notre approche s&rsquo;int\u00e8gre parfaitement aux protocoles de r\u00e9\u00e9ducation existants, ce qui facilite son d\u00e9ploiement dans diff\u00e9rents environnements cliniques. \u00bb<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Une nouvelle \u00e8re pour le r\u00e9tablissement de la mobilit\u00e9<\/h4>\n\n\n\n<p>Cette fusion r\u00e9volutionnaire de la neuroproth\u00e9tique et de la robotique repr\u00e9sente une avanc\u00e9e majeure dans la r\u00e9\u00e9ducation des paralys\u00e9s.
